Summit is an Out-of-Network provider for Insurance companies in Utah. This means that we do not directly take insurance plans. WE DO ACCEPT HSA PLANS or HMA PLANS.

Your insurance provider will always be your best source of direct benefits and coverage questions for what is allowed in your plan. Reimbursement rates vary for each insurance carrier.
We happily provide a superbill for you to submit to your insurance following treatment for your reimbursement, or you can simply use your HSA / HMA account plans for payment. We take all other major Credit Cards as well for payment via a secure payment method.

HSA Plans are accepted and we will provide a superbill for your reimbursement. Please give us a call and we'd be happy to work out a plan for you.
Please note that payments for all Hyperbaric Oxygen Therapy (HBOT), Ozone, Dry Needling, Corrective Exercise & Rehabilitation, Taping, Laser, and Diagnostic Ultrasound are due at the time of service. So that we may better serve you and provide best possible treatment, we do not accept insurance directly. You will be provided with a coded bill to submit to your insurance company for reimbursement. The amount of reimbursement varies among insurance providers. For non-insurance-covered services, we accept HSA and FSA Cards (American Express, Mastercard, Visa, and Discover).
